Policies, Procedures and Patient Information

Lbutton.gif (938 bytes) Appointment Information:  Please call ahead at 383-2693.  We recognize the value of your time and make every effort to maintain your appointment as scheduled.  Emergencies do arise, however, and surgeries may occasionally delay your appointment time.  When these circumstances occur, we appreciate your patience and understanding.  Please feel free to contact our office on the day of your appointment to avoid any unnecessary waiting.

 

Lbutton.gif (938 bytes) Your Telephone Call Is Important (383-2693): Feel free to call us when you have questions or concerns.  when you call your physician may be seeing patients in the clinic or may be in surgery.  Your call may be returned after 4:00 p.m. on the day it it is received.

 

Lbutton.gif (938 bytes) Prescription Refills: To safeguard your health, we issue prescriptions and refills only during office hours.  Please try to call 2 or 3 days before you run out of medicine if you anticipate needing a refill.  Medication refills and new prescriptions will not be given after hours or on weekends.

 

Lbutton.gif (938 bytes) Release of Medical Records: Should you ever need  have your medical record released, a written request signed by the patient or patient guardian is required to ensure release of information in a timely manner.  A standard release form is available from our Medical records Office.  Please allow 5-7 working days for processing of records.
Lbutton.gif (938 bytes) Fees and Insurance: Payment in full is expected at time of your visit, unless prior arrangements have been made.  Exceptions include pre-paid insurance plans, such as preferred provide programs (PPOs), Health Maintenance Organizations (HMOs), point of service products, and independent practice associates (IPAs).  We accept Visa, Master Charge, and American Express as well as cash.

It is the responsibility of the patient to follow the referral guidelines of your health coverage carrier before seeking specialty care.  The Lipscomb Clinic will look to the patient for deductible and coinsurance payments.

Insurance claims will be filed for the health insurance plans in which our clinic is a participant.

Please call us if you are in doubt as to whether your insurance plan is one in which we can participate.

When The Lipscomb Clinic files claims for benefits or services rendered, benefits are assigned to The Lipscomb Clinic.  Please remember that insurance coverage is a contract between the patient and the health coverage carrier.   The Lipscomb Clinic will look to the patient for payment in full of any balance.

If you are being treated as the result of an injury, we require a detailed description of the injury and how it occurred. 

Many Insurance plans now require pre-certification or prompt notification upon entering the hospital.  Compliance with the requirements of your plan is your responsibility.  Please let your physician know if your plan requires pre-certification.
